Dosage and Administration: The recommended dose of paliperidone is 3mg to 6mg/day, taken in the morning, with or without food.
Adverse Reactions: Common adverse reactions include restlessness, extrapyramidal symptoms (dyskinesia), tachycardia, and drowsiness. Paliperidone is an atypical antipsychotic that may increase mortality in elderly patients with dementia-related psychosis; therefore, it should not be used in patients with dementia-related psychosis.

Mechanism of Action: Paliperidone is the major active metabolite of risperidone. The mechanism of action of paliperidone in the treatment of schizophrenia is unknown but is thought to result from a combination of antagonism at central dopamine D2 and serotonin 5HT2A receptors, as well as H1 histamine receptors. Paliperidone has no affinity for cholinergic or α-/β-adrenergic receptors. The pharmacological effects of the enantiomers (+)- and (-)-paliperidone are similar in vitro.
