01 Independent Legal Entity Inspection and Testing Institutions

(Foreign-funded institutions shall refer to this item)

"Application Form for Qualification Accreditation of Inspection and Testing Institutions" (1 copy, with electronic text in the format published on the Provincial Quality Supervision Bureau website);

Business license of the legal entity, institutional legal person certificate, or other legal entity proof documents (the business scope must include inspection and testing services);

In regions where the commercial registration system reform has been implemented, the business scope information shall also be submitted (1 copy, original required for verification);

Organization code certificate (1 copy, original required for verification; in regions where the business license and organization code certificate are combined into one certificate, only the business license is required);

Proof of fixed premises ownership/usage rights;

Proof of labor relations for professional technicians and management personnel, i.e., social security proof stamped by the social security department (1 copy, original required for verification, with social security proof for three months or more required);

Proof of independent allocation of inspection equipment (1 copy, original required for verification);

Typical inspection and testing reports or certificates (one each; applicable to first-time and expanded scope applications, additional copies may be provided as required by the evaluation team leader);

Records of internal audits and management reviews of the management system (applicable to first-time and reassessment evaluations);

Qualification certificates for personnel engaged in inspection and testing in special fields.

All the above materials must be stamped with the official seal of the inspection and testing institution (legal entity).

02 Non-Independent Legal Entity Inspection and Testing Institutions

In addition to the materials required for independent legal entities, the following materials must also be prepared:

Legal authorization document (1 copy)

Approval document for the establishment of the inspection and testing institution (1 copy)

Appointment document of the top management (1 copy)

All the above materials must be stamped with the official seal of the inspection and testing institution (legal entity).
